The following report forms have been sent to all Bethels:

3 Copies of Form 110A, Annual Report
1 Copy of Exhibit 5
1 List of your Bethel membership
1 Annual Dues Increase Chart (revised)

You must do the following:

Fill out the 3 copies of your Annual Report, keep one copy and send the other two copies

          The Grand Secretary.

Fill out Exhibit 5 for all donations made to your Bethel of over $250.00 and return that

          Form to the Grand Secretary whether or not you have had any donations.

Check over your membership list and update it by adding new members, noting Majority

          Members, and changing addresses where necessary, and return the list to the
          Grand Secretary.

The Constitution and ByLaws were amended at the 2005 Supreme Session to

          Change the payment of the insurance fee. The annual fee for each member
          of the Bethel for the payment of liability insurance shall be the latest actual
          premium paid, plus ten percent (10%) divided by the total membership in
          the USA as of the last Annual Report.

Figure our your remittance due to the Grand Guardian Council. Make your check payable

         to JDI-GGC OF MN and send it with your reports to the Grand Secretary.

ALL OF THESE MUST BE BACK TO THE GRAND SECRETARY BY JANUARY 31.
